Fractional Chief Technology Officer

Healthcare Software Engineering CompanyPosted Oct 19, 2026

Job Description

Fractional / Contract Chief Technology Officer

Location: Remote

Reports to: Chief Executive Officer

The company is engaging a fractional or contract Chief Technology Officer to finalize technical scope, oversee a development partner, and provide delivery assurance. The role will work with the CEO and development partner to establish an agreed implementation baseline, define measurable milestones and acceptance criteria, and oversee execution against the approved scope, budget, and schedule. Applied expertise in Bayesian modeling is a core requirement.

Responsibilities

Initial mandate — Scope finalization and delivery planning

  • Review existing product specifications, technical assets, and the development partner’s proposed scope, architecture, staffing, and estimates.
  • Reconcile the proposal with company requirements, identifying omissions, ambiguous commitments, dependencies, and unresolved decisions.
  • Work with the CEO and development partner to finalize deliverables, responsibilities, exclusions, sequencing, and acceptance criteria.
  • Establish milestones with accountable owners, target dates, measurable thresholds, and required evidence for acceptance.
  • Assess the feasibility of the delivery plan, including resources, integration dependencies, clinical review, data readiness, and security requirements.
  • Establish change control for proposed additions, removals, or substitutions, documenting effects on cost, schedule, and functionality.
  • Prepare a consolidated scope and delivery plan for CEO approval.

Ongoing mandate — Technical leadership and partner accountability

  • Lead technical reviews and independently assess development progress, quality, and delivery risk.
  • Review key architecture decisions, code, model implementations, test results, and documentation.
  • Verify milestone completion against agreed acceptance criteria and document deficiencies requiring correction.
  • Maintain scope discipline, ensuring agreed functionality is delivered and proposed changes receive appropriate review and approval.
  • Evaluate change requests and distinguish additional work from obligations already included in the agreed scope.
  • Anticipate bottlenecks, including access to integrations, data readiness, specialist availability, testing capacity, and infrastructure dependencies.
  • Maintain a risk and decision register with accountable owners, mitigation actions, and resolution dates.
  • Provide concise reporting on milestone status, forecast completion, budget exposure, and decisions requiring CEO input.
  • Oversee release readiness and handoff, ensuring the company controls its repositories, accounts, infrastructure, documentation, and technical work product.

Modeling and clinical-system oversight

  • Independently assess the development partner’s mathematical approach and implementation for personalized statistical models.
  • Review population-level assumptions, individual model state, model structure, longitudinal updates, correlated observations, missing data, and uncertainty.
  • Reproduce key calculations and evaluate reference implementations, diagnostics, calibration, and predictive-validation evidence.
  • Confirm that edited, deleted, or backdated observations are handled consistently and that model behavior remains reproducible.
  • Ensure deterministic safety rules take precedence over personalized model outputs.
  • Coordinate clinical approval and independent statistical review where required.
  • Oversee the technical integrity of historical data imports, clinical summaries, healthcare interoperability, and clinical terminology mappings.

Security, regulatory, and coordination

  • Coordinate security and regulatory specialists and translate their requirements into documented engineering work.
  • Anticipate and mitigate delivery risks related to security, privacy, and regulatory requirements.

Communication and decision support

  • Be candid, prepared, and collaborative; challenge technical assumptions and delivery claims with evidence and present clear options.
  • Provide actionable recommendations to the CEO and communicate directly about risks, scope changes, and delivery status.

Qualifications

  • Demonstrated experience personally building, implementing, or independently validating hierarchical Bayesian models with longitudinal or sequential updating.
  • Ability to inspect modeling code and reproduce results using relevant statistical programming tools.
  • Senior technical leadership experience, preferably including fractional CTO engagements with founder-led companies.
  • Experience refining software scopes and managing external development partners against defined budgets, schedules, and acceptance criteria.
  • Healthcare software experience involving sensitive clinical data, secure architecture, interoperability, and clinical terminology.
  • Strong technical judgment across mobile applications, APIs, backend services, cloud infrastructure, testing, and deployment.
  • Experience identifying delivery risks early and implementing practical mitigation plans.
  • Ability to challenge technical assumptions, support recommendations with evidence, and present clear options to executive leadership.
